Tonic Co Pty Ltd t/as Shannon Stokes Naturopath ABN 49 642 498 557 (“Tonic Co” “us”, “we”, “our”), is committed to providing quality services to you and this policy outlines our ongoing obligations to you in respect of how we manage your Personal Information.

 We comply with the Privacy Act 1988 (Cth) (“Privacy Act”) and the applicable Privacy Principles in the Privacy Act when handling personal information, together with the Public Health Act 2005 when handling personal information which is health information. We have also adopted the Australian Privacy Principles (APPs) contained in the Privacy Act. The APPs govern the way in which we collect, use, disclose, store, secure and dispose of your Personal Information. WHAT IS PERSONAL INFORMATION AND WHY DO WE COLLECT IT?

  • From our customers – when they make a booking on our Website www.tonicco.com.au request an assessment for the purposes of dietary or health advice, purchase a nutritional plan or nutritional products, or otherwise in the course of providing our products or services at any other time from time-to-time;

  • When we receive enquiries through the Website or when the Website is otherwise accessed or used;

  • When we receive enquiries via email; and

  • From third party service providers – when they are engaged to supply Services to us (such as professional services), provide information to us or invoice us for the provision of services.

The kind of Personal Information we may collect will depend on who you are (e.g., a client or third-party service provider) and the nature of your interaction with us and may also include health information.

We may ask for your full name, email address, postal or residential address, age, weight, gender, any medical conditions or allergies/intolerances, nutritional supplement use, billing information and other Personal Information. If you do not provide us with all the information we request, we may not be able to provide our Services and or recommend any Products.

Some Personal Information we collect may include health information. Health information includes any information collected about your health or disability, and any information collected in relation to a health service you have received (“Health Information”). We collect Health Information from customers for the purposes of providing naturopathic care, dietary advice, meal plans and/or keeping a record of the Products and/or Services you have purchased from us. This enables us to provide our Services and assess the needs of our customer in the provision of our Services and to generally assist in optimising the quality of our service safely, appropriately, and accurately.

SENSITIVE INFORMATION

Sensitive information is defined in the Privacy Act to include information or opinion about such things as an individual's racial or ethnic origin, political opinions, membership of a political association, religious or philosophical beliefs, membership of a trade union or other professional body, criminal record, or health information.

Sensitive information will be used by us only:

  • For the primary purpose for which it was obtained

  • For a secondary purpose that is directly related to the primary purpose

  • With your consent, or where required or authorised by law.

THIRD PARTIES

Where reasonable and practicable to do so, we will collect your Personal Information only from you. However, in some circumstances we may be provided with information by third parties. In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.
